For 2011 Health Care Flexible Spending Account (FSA) Participants With a Remaining Balance

  • SDSU Research Foundation changed its Third Party FSA Administrator from TRI-AD to Discovery Benefits as of January 1, 2012. To avoid confusion, and as is standard industry practice when an employer changes their FSA Administrator, your Flex Card (debit card through MasterCard) issued by TRI-AD was de-activated on 12/31/2011.
  • You are no longer able to use TRI-AD’s Flex Card (debit card through MasterCard) for services incurred after December 31, 2011, even if the services were or are performed for eligible charges, and even if you enrolled in an FSA for 2012.
  • TRI-AD will continue to administer your remaining claims against your 2011 FSA balance. If you have a balance left in your 2011 Health Care FSA, you may submit any eligible expenses that you incurred out-of-pocket between January 1, 2012 and March 15, 2012, to TRI-AD by submitting a claim form with proper documentation by March 31, 2012. Personalized claim forms are available to you online. You can log into TRI-AD's website and download a form that is pre-printed with your personal information. You may also mail or fax your claims for reimbursement. TRI-AD’s fax number is 866-233-4741.
  • Manage your remaining 2011 FSA account online through www.tri-ad.com.
  • If the claim amount is more than the balance left in your 2011 account and you elected a 2012 FSA, you may submit the balance of the expense to Discovery Benefits for reimbursement.
  • To use your remaining 2011 health FSA balance before you lose it, review the following ideas:

    1. Get annual check-ups for your eligible family members: Even if you think you’re doing just fine, prevention is better than waiting until you notice a big symptom.
    2. Get vision exams for your eligible family members: If you or your family members wear contacts or glasses and need new ones, have your eyes examined or order new contacts or glasses (consider prescription sunglasses too!)
    3. Think about dental check-ups or teeth cleanings for your eligible family members
