Health & Dental Benefits for 2012
- Dental ID Cards: Your new dental plan provider, United Concordia, has mailed your ID cards to your home. If you enrolled dependents, you received two ID cards with your name listed, even if you enrolled more eligible dependents. Please note all cards are issued in the name of the employee only. If you need more than the two ID cards that were mailed to you for other family members, you may request them online by going to www.ucci.com or you may ask SDSURF’s Benefits Team to make the request for you.
- Go Paperless: Sign up for My Dental Benefits and receive your explanation of benefits (EOBs) online, order additional ID cards, view claims, etc.
- Submitting Dental Claims: Your dentist may use United Concordia’s two new online tools: online electronic claims submission and payments via electronic funds transfer (EFT). If you are enrolled in the PPO and need claim forms to submit your dental charges to United Concordia for reimbursement, you may download one from their website and provide to your dentist office for processing. If your dentist uses a Universal Claim form, PPO claims may be submitted to:

- Other Changes: If you switched health plans, enrolled in a Flexible Spending Account, or added dependents to your health or dental coverage, your new coverage became effective January 1, 2012. You should have received ID cards from your carriers by now. If you have not, or if there are corrections needed, please email your SDSU Research Foundation benefits team at benefits@foundation.sdsu.edu.
- Save on Prescriptions: To save on prescriptions throughout the year, consider using Kaiser or Health Net’s mail order pharmacy for maintenance medications.
- Changing Primary Care Physicians: You may change your primary care physician anytime by calling your medical plan’s member services phone number. Call Kaiser at 800-464-4000, or call HealthNet HMO at 800-522-0088.
