Choosing Between DeltaCare USA and the PPO
To determine which plan is best for you and your family:
- View this Delta brochure for a comparison (PDF Format, 17KB)*
- DeltaCare USA: You'll pay a specific and predetermined copayment for procedures (PDF Format, 25KB)* (on pages 9-18) but will need to select a DeltaCare USA dentist.
- PPO: You'll pay a percentage of the dental bill but will be able to receive treatment from any dentist. You will pay less out of pocket if you receive treatment from a Delta Premier Dentist and even less if you receive treatment from a Delta Preferred Dentist.
- Find out which plan your dentist participates in by viewing the Dentist Directory.
Allowable Changes at Anytime: No Forms Required!
- Your DeltaCare USA Dentist
You are able to change your DeltaCare USA dentist at any time throughout the year by calling 1–866–444–0187. Dial 2. - Between the DeltaCare USA plan option and the PPO at Anytime
You can change between the DeltaCare USA and the PPO at any time. It's easy:- Call Delta's dedicated SDSURF customer service line at 1–866–444–0187
- Dial 1 if you have a PPO question
- Dial 2 if you have a DeltaCare USA question
- Dial 3 if you want to switch from one plan to the other
- Your change will be effective based on when you call to make the change. If you make the call to change between the 1st and 15th of the month, the change will be effective on the 1st of the following month. If you make the change between the 16th and last day of the month, it will be an additional month before the change is effective.

Get Your Dentist on Delta's Network
If your dentist is NOT currently a Delta Dentist, you may nominate them for membership. Submit a nomination form to Delta. The Delta staff will contact your dentist directly with membership information.
