I'm not happy with my dentist, can I make a change?

The great news is yes, you can switch dentists at any time without waiting for Open Enrollment. It's as easy as calling Delta's Membership Services at 866–444–0187. You can also switch dental plans at anytime as well by calling the same number and choosing option 3. Please notice, any changes made between the 1st and the 15th of the month will be effective the 1st of the next month. If you call between the 16th and the end of the month, the change will be effective the first of the next following month. For example: Changes made between January 1 and 15 are effective February 1st. Changes made between January 16 and 31 are effective March 1st.

I plan on taking a leave of absence, what do I need to do?

The best thing to do is to notify Human Resources at 619–594–4139 at least 30 days in advance, if possible. An assigned HR Leave Administrator will discuss the types of leave of absence you are qualified for and will answer any general questions you may have. You will receive a packet with detailed instructions and several forms that will require your review, supervisor's approval, and/or physician's certification. Your packet also contains SDSU Research Foundation's leave guidelines as well as general information about your rights under FMLA (Family Medical Leave Act) and CFRA (California Family Rights Act). I just recently got married, what forms do I need to complete?

Congratulations! There are several steps you will need to take depending on your individual situation. Some employees may only need to complete name change forms while other employees may need to complete name change and address change forms. Please review the Newly Married Forms and complete all forms that relate to you. Please notice that if you decide not to add your spouse to benefits coverage because he/she has there own coverage through their respective employer, completion of a Medical Declination form will be required. In addition, supporting documents such as an active Drivers License and Social Security card for all name changes is required to be viewed by HR.

I would to like add money to my retirement plan?

Review all the information contained in SDSURF's Voluntary Tax Deferred Annuity Program brochure  (PDF Format, 90KB)*. Decide which investment company you would like your contributions to go to. Once decided, you will need to set up an account with that company. Each investment company will have its own enrollment instructions. After you have set up an account with them, notify HR of the enrollment, complete a Salary Reduction Agreement for Voluntary Tax-Deferred Annuity Program  (Word Format, 402KB)* and return the completed form to Human Resources.

How long does COBRA last and how much does it cost?

COBRA continuation coverage is temporary. The length of COBRA coverage will depend on your qualifying event. If the qualifying event is death of employee, enrollment in Medicare, divorce, or dependent child losing eligibility as dependent child, COBRA is up to 36 months. For all other qualifying events such as termination of employment or reduction in hours, COBRA may be continued for up to 18 months.
