This notice is designed to comply with the requirements of the Women’s Health and Cancer Rights Act of 1998, and is required to be provided upon enrollment in one of SDSU Research Foundation’s group health plans, and annually thereafter, under the provisions of Public Law 105-277, Section 902.
Group health plans, health insurers and HMOs that provide medical and surgical benefits for mastectomy must provide certain benefits related to breast reconstruction. If a participant or beneficiary who is receiving mastectomy benefits and who elects breast reconstruction in connection with the mastectomy, must be provided with coverage that includes:
Coverage must be provided in a manner determined in consultation between the attending physician and the patient. This coverage may be subject to deductibles and coinsurance limitations consistent with those established for other benefits under the plan.
