Make Your Changes Now Through November 15, 2011

Open Enrollment is your annual opportunity to review and update your health and dental plan benefits to meet your family's needs.

During Open Enrollment you may:

  1. Review what's new in 2012
  2. Enroll or re-enroll in a Flexible Spending Account (FSA) for health care and dependent care expenses for 2012.  Use the Benefits Enrollment/Change/Decline Form (PDF Format 590 KB)** to enroll or re-enroll in an FSA. 
  3. Change health plans
  4. Enroll in or drop coverage for health or dental insurance
  5. Add or remove dependents (including domestic partners) from coverage
  6. Purchase additional life and accidental death and dismemberment
    insurance for you and your family

All changes made during this year's Open Enrollment are effective January 1, 2012.


What Action Do I Need To Take Before November 15, 2011?

  1. If you are enrolled in Delta Dental, you must complete a new enrollment form (PDF Format 590 KB) ** electing dental coverage through SDSURF's new dental carrier, United Concordia (UCCI). You may enroll in UCCI's DHMO plan option or the PPO option.  Your plan option election will remain for the year.  You may not switch between the options during the year. To help make your decision, please review the dental plan summaries and frequently asked questions in the What's New section of this website..  
  2. No action is required if you are enrolled in Kaiser, Health Net HMO, or Health Net PPO and want to stay with the same carrier and coverage that will renew in 2012.
  3. If you want to participate in the Flexible Spending Account (FSA) in 2012, you must submit a new Benefits Enrollment/Change/Decline Form  (PDF Format, 590 KB), **even if you are currently enrolled.  The annual medical FSA maximum you can contribute in 2012 will increase to $3,000.
  4. If making changes, download the appropriate form(s), complete, and send by Friday, November 15, 2011, to:
