Lost or Stolen Card
In the event of a lost or stolen card, immediately contact a customer service representative at JPMorgan Chase at:
1-800-270-7760
After contacting JPMorgan Chase to cancel the account, the cardholder must complete a Cardholder Maintenance Request form and obtain AO and PI approval in order to have a new card issued.
