State regulations require a principal investigator to disclose whether or not he or she has a direct or indirect financial interest in the project's sponsor. If the sponsor is a private, for-profit organization, a Statement of Economic Interest (Form 700U) must be submitted to the Conflict of Interest Committee via San Diego State University (SDSU) Research Foundation. Principal investigators will not be able to spend on awards received from private, non-profit organizations unless a form has been completed and reviewed by the Conflict of Interest chairperson and/or committee. Any person who will work on an NSF or DHHS project who is responsible for the design, conduct, or reporting of work performed on the project must complete a Financial Interest Disclosure statement. This disclosure includes employees of SDSU, SDSU Research Foundation, and all subcontractors. The completed disclosure form must be brought to SDSU Research Foundation at the time of the proposal submission. Principal investigators will not be able to spend on awards received from NSF or DHHS unless a form has been completed. A protocol must be submitted to the Institutional Review Board (IRB). Information regarding the use of human subjects is posted at the IRB website, located at http://www-rohan.sdsu.edu/~gra/grad/research/hrpp.html. In addition, all investigators using human subjects must complete the web-based tutorial and assessment: http://www-rohan.sdsu.edu/~gra/login.php. The Institutional Animal Care and Use Committee (IACUC) is responsible for ensuring that adequate care and facilities for vertebrate animals are available. Protocol forms may be obtained by calling ext. 40905 and are posted at http://www-rohan.sdsu.edu/~gra/grad/research/animalcare.html. In addition, all investigators and personnel working on a project using animals must complete the necessary Animal Health and Welfare Training. IACUC verification is required if proposal is funded.
The following four regulated materials require environment, health and safety review. If the proposed research involves any of these regulated materials, an Environment, Health and Safety questionnaire must be completed and submitted with the Proposal Routing Form. Prior to working with any of the regulated materials described below, additional authorization application forms must be completed, submitted and approved accordingly. The EH&S Department is responsible for ensuring that research or experiments involving controlled substances or listed chemicals complies with Drug Enforcement Agency regulations. Prior to possessing or working with DEA controlled substances, a Controlled Substance Use Authorization application must be completed and submitted to EH&S. Prior to possessing or working with DEA listed chemicals, a List Chemical Use Authorization application must be completed and submitted to EHS. The Institutional Biosafety Committee (IBC) is responsible for ensuring that recombinant DNA research or experiments involving biohazardous materials are conducted in compliance with NIH, CDC Guidelines, and Cal OSHA Requirements to promote safe and responsible work practices. Prior to possessing, storing, or working with or transporting infectious agents, select agents and toxins, human or nonhuman primate materials (including blood, body fluids, cells and tissues), recombinant DNA or transgenic animals, a completed Biological Use Authorization (BUA) application form must be submitted and approved by the IBC. The BUA application form, regulatory requirements and guidelines may be found online at the EH&S web site, http://bfa.sdsu.edu/ehs/forms.htm. The EH&S Department is responsible for ensuring that research or experiments involving regulated or listed carcinogens and acutely or extremely hazardous chemicals are stored, handled and disposed of safely as required by local, state or federal regulations. Prior to working with the regulated chemical a Standard Operating Procedure (SOP) describing the storage, use, and disposal of regulated or listed carcinogens and acutely or extremely hazardous materials must be submitted to EHS. The SOP template is posted at http://bfa.sdsu.edu/ehs/forms.htm. Material Safety Data Sheets (MSDS) of these chemicals must be on file in the laboratory. The Radiation Safety Committee (RSC) has the ultimate responsibility for the use of radioactive materials and radiation-producing machines at SDSU, and shall be the reviewing and authorizing agent for the use of all radiation for university projects. Principal investigators who plan to work with radioactive materials or radiation-producing machines must submit a completed application posted at http://bfa.sdsu.edu/ehs/pdf/ruaappl.pdf along with a statement of training and experience (see http://bfa.sdsu.edu/ehs/pdf/trainexper.pdf) (PDF format, 235 KB)* to the Radiation Safety Officer (RSO) in the EH&S Department. Applications may also be obtained by calling ext. 46879. The RSO will review the application for completeness and applicability with license conditions. The Diving Safety Officer (DSO) under the direction of the SDSU Diving Control Board (DCB) is responsible for ensuring that all scuba diving under the auspices of SDSU is conducted in a manner that will maximize protection of divers from accidental injury or illness. All projects involving divers are to be conducted in accordance with the SDSU Diving Safety Manual and the American Academy of Underwater Sciences Standards for Scientific Diving. Principal investigators must submit a proposal outlining diving protocols to the DSO for approval. The DSO (ext. 46799) is available to answer any questions concerning diving requirements, proposal contents, or the diving project approval process. Principal investigators must include a proposal outlining: (1) diver's qualifications; (2) approximate number of proposed dives, (3) location(s) of proposed dives; (4) estimated depths and bottom times anticipated; (5) decompression status and repetitive plans, if required; (6) proposed work, equipment and boats to be employed; (7) any hazardous or potentially hazardous conditions anticipated; and (8) when diving out of the southern California area, emergency procedures including the nearest Hyperbaric Treatment Facility and the available means of transportation. If a project includes travel or work in a foreign country, diving, high-risk tasks such as piloting (or being a passenger in) small aircraft, mountain-climbing in remote areas, underwater research, working aboard vessels, transporting groups of people in automobiles, etc., or any students working on these tasks, the SDSU Research Foundation Risk Manager will review for adequate safety protocols and insurance coverage. NOTE: Space planning normally requires at least a three-month lead time, so it is important to meet with facilities planners as soon as there is an informal indication from the funding agency that the project will be funded and an expected start date has been agreed upon.
Cost sharing should not be proposed on any proposal unless it is required by the sponsor agency. All cost sharing proposed must be approved by the appropriate SDSU dean or administrator before the proposal is mailed. The type and amount of cost sharing must be itemized. Letters from units or entities providing cost sharing may need to be secured. Any cost sharing offered must be documented. Federal funds may not be used to cost share on other federally-funded projects. Cost sharing from third parties must be confirmed with a fully executed Cost Sharing MOU.
