Health Risk Assessments with Kaiser Permanente

Summary
All employees who have selected Kaiser Permanente as their health care provider, are invited to an interactive session about Kaiser Permanente online resources, and to take a Health Risk Assessment. The Health Risk Assessment (HRA) is a brief questionnaire designed to identify potential health risks, outline specific information, and receive an action plan to assist you in making healthful changes that will impact your health and prevent chronic disease. Once your HRA is complete, depending on your outcome, you may be contacted by a health care professional for a follow up visit to determine if you are at risk. The HRA is a great tool to help you on your way to better health.
